General advice for all patients

Do you have any other advice you'd like to share with patients in general? I think I would say you know take responsibility, take ownership for your health care, work with a team. The world's developed, it's no longer a situation where the doctor tells the patient what to do, it's a partnership. Doctors will give you advice but we need you to buy into that, to help us by taking control of your diabetes, trying to lose weight, I know it's difficult. The other key bit of advice I say to every patient is please put a list of your current medications in your wallet so that wherever you go you always have that list with you. If you get hit by a bus tomorrow, taken into accident and emergency in a random hospital, No one knows what medication you're taking. There is this myth that we have access to all your information. We don't. If you go and see your GP and they change your medications, we won't know that when you come to the kidney clinic. So please, please, please bring a list of your medication or the medications themselves with you to every hospital appointment.
