Wide Local Excision of Breast

Miss Sarah Tang explains risks, benefit and reasons why re-excision is sometimes necessary following Wide Local Excision for breast cancer.

Breast screening aims to find breast cancers early. It uses an X-ray test called a mammogram that can spot cancers when they are too small to see or feel.

Filmed on 13/01/2016 KK, 53 years old female, suffering from obesity. Few years ago she underwent a Heliogast band as a treatment for obesity. She lost some weight, but started gaining again. Apart from that she did not tolerate band well, was feeling sick and attended few times to Emergency Department. Her medical history includes high blood pressure, depression, ovarian cysts, and removal of the gall bladder laparoscopically (keyhole surgery). She was offered laparoscopic removal of the band and gastric bypass. Before planned procedure underwent psychiatric assessment, which is a part of work up before bariatric procedures. After the procedure she developed chest infection which was treated with antibiotics. Otherwise she recovered reasonably well and started losing weight in a steady fashion.

Filmed on 26/01/2016 RK, 58 years old female, suffering from obesity with body mass index (BMI) above 40. She could not maintain the weight loss achieved with diet and tablets. Her medical background covers pre-diabetic condition and thyroid cancer. She was offered laparoscopic (keyhole surgery) gastric bypass. Before the procedure underwent mandatory psychiatric assessment. Recovered well after the surgery. However, had persistent lower abdominal pain. CT of her abdomen was normal and as a part of assessment was offered a diagnostic laparoscopy, during which scar tissues (adhesions around small bowel were found. After the surgery she lost 27% of total body weight. However, she continued to have minor pain in the left side of the abdomen and abdominal distension.

Filmed on 13/01/2016 AK, 46-year-old female, suffering from morbid obesity with body mass index (BMI) above 46. She tried many diets and failed to sustain the weight loss for longer period of time. Her medical history covers high blood pressure and diabetes. She was offered laparoscopic (keyhole surgery) sleeve gastrectomy (removal of the part of stomach body), which would give her 25% weight loss. She underwent mandatory psychiatric assessment before the procedure. Her procedure went very well. She lost 20% of total body weight within few months’ following surgery. This allowed her to remain only on oral tablets for her diabetes and insulin could be stopped. Also her blood pressure improved much and her medications were reduced.

She could not sustain a weight loss achieved with diets and tablets. She was offered a laparoscopic (keyhole surgery) gastric bypass. Before the procedure underwent a mandatory psychiatric assessment. She recovered well from the procedure. Started losing weight in a steady fashion and her mobility improved as well since the procedure.
