Would you commonly use a bypass machine in open aneurysm repair? So not for the most common. So the most common are abdominal aneurysms. And under those circumstances, you can clamp the aorta and have the bottom half of the body completely out of circuit. And you've got literally hours, so four or five hours before you start getting into problems with poor blood supply to the legs. and there is ample time to do a relatively straightforward aneurysm. There are aneurysms which extend into the thoracic cavity and even into the artery aorta, and these are more complex because they are then supplying some really important organs that cannot tolerate four or five hours without blood supply, for example the brain, for example the small bowel, the guts and the kidneys. So as you go more proximal, higher up in the body to clamp, you do then need support to these other organs. And one of the ways of doing that is with a bypass machine. For open surgery, the more difficulties you've got with that is you have to thin the bloods to be on the bypass machine, so patients have to be antifragilated. And the more VA water is exposed and the more cavities of the body which are opened to get to that exposure, the more bleeding one has. So one of the big complications of thoracal abdominal surgery are aneurysm affecting the whole of the abdominal aorta and the asthmatic aorta, for example, is bleeding and that's managed by a combination of means including cell salvage to recirculate the body's blood loss into a machine and then it's given back to them but even with that it's not to be underestimated the amount of blood loss which can occur in a long complex procedure like that and those patients have to be fit to sustain that so there is not a non-operation one can do in an 80 year old but it is a good option for the younger especially patients with connective tissue disease who you hope have got a very much longer lifespan and they are of an age that they can tolerate that magnitude of surgery.
