Who should be following you up if you have Heart Failure and diabetes?

Your GP can follow you up if you have a diagnosis of heart failure and diabetes. There may well be times where you need to see a specialist, either a specialist in heart failure or a specialist in diabetes, especially if the diabetes is not well controlled or if your heart failure symptoms are getting worse. Ideally most people with heart failure should be seen at least once by someone with a specialist expertise in heart failure so that they can make sure you're on all the different available treatments and that they've understood the exact cause of your heart failure.
